An occupational injury is any adverse effect or damage to faculty, staff, and Institute employed students resulting from working that can be identified by time and place. General & Occupational Safety has developed forms and guidelines to follow when reporting and handling an injury or illness case. All incidents should be reported, please follow procedures listed below for specific reporting instructions:

The supervisor completes the Supervisor Incident/Accident Reporting Form and submits it within 24 hours of knowledge of the incident.
The employee completes the Employee Incident/Accident Reporting Form and submits it within 24 hours of the incident.
