1. Login to EHSA.
  2. Click on "Inspections" in the top menu bar.
  3. Click "Equipment Inspection Results".
  4. Click "+Add", then on the screen that appears:
    • Select "Xray" from the "Device Type" drop menu
    • Leave "Category" as "X-Ray"
    • In the "Equipment" drop menu, select the radiation machine inspected. The listing includes the serial number, model, room, and building.
    • Select the date the inspection was conducted in "Inspection Date"
    • Leave the "Status" field as "Pending"
    • Enter the inspection date in the "Date Completed" field.
    • The "Inspection Notes" field is used to document items that are useful information or recommendations that aren't already noted in the "Observations" field of an "Unsatisfactory" inspection item. This "Inspection Notes" field will display on the cover letter.
    • Select the "Lead Inspector"
    • Click "Save and View Checklist"
  5. Eleven inspection subcategories will display.
    • General Notes for all Subcategories
      • Inspection items tagged with "- Info Required" must be opened and information entered.
      • Inspection items tagged with "- Info Required if Unsatisfactory" must be opened and information entered/selected if the item is found to be unsatisfactory.
    • Operating Conditions
      • Leave the "Answer" column blank for each of these items.
      • You must open each item and type in the relevant information.  
    • ORS Survey Instrumentation
      • Leave the "Answer" column blank for each of these items.
      • Record the survey meter/ion chamber used in this subcategory. If one of the meter types isn't used, leave it blank. If more than two survey meters of a given type are used, document the additional meter(s) in the "Observations/Notes" field when you open that inspection item.
    • General Notes for the Remaining Subcategories
      • The "Answer" column has five options in the drop menu. Please use them as indicated below.
        • Satisfactory - self-explanatory
        • Unsatisfactory - self-explanatory
        • Undetermined - This is typically used when a device is Out of Service, so the item wasn't evaluated
        • Not Applicable - Use this for questions in a subcategory that applies, but for some reason that question doesn't apply to the given instrument. For example, under the "Analytical" subcategory, there is an item called "Tube head posted". There are some analytical devices where the tube head is inaccessible, so it can't be checked. In this case, "Not Applicable" would be the answer, with a note made under "Observations/Notes" that the tube head is inaccessible. Another example, some devices have no shutter, so the "Shutter functional" item would be listed as "Not Applicable". These items will show up on the inspection report checklist.
        • N/A - Use this for all the items in subcategories that don't apply. For instance, if the inspection is for an XRD, you would click the button "Set Subcategory to N/A" for the categories that don't apply (such as Byproduct or Cabinet). These items will not show up in the report.
    • All Devices
      • This is the section that is filled out regardless the type of device being inspected.
      • Be sure to use "Not Applicable" as the answer for items in this subcategory that don't apply to the device.
    • Analytical, Analytical Open Beam, Cabinet, Open X-Ray, Byproduct, Non-Medical Fluoroscope, Non-Medical Radiograph, and Accelerator are the remaining subcategories. Click the button "Set Subcategory to N/A" for those that don't apply.  
      • There are no subcategories for the medical and dental devices. The spreadsheets that are used to document the requirements for each will need to be attached to the cover letter and inspection page that include the Subcategories Operational Conditions, ORS Survey Instrumentation, and All Devices.
