These instructions describe how to add a laser to the laser inventory and attach it to the Laser Supervisor's laser permit.  The information to be entered comes from the Form LR-1, Laser Regsitration submitted by the Laser Supervisor.

  1. Click "EHSA" > "Equipment"
  2. Under the "Lasers" section, click "Lasers"
  3. Click "+Add"
  4. Enter the laser "Serial #"; if no serial number is present on the laser, create one with the format PICodeNOSN01, where 01 is incremented to 02, 03, etc.
  5. Enter the manufacturer name in "Make"
  6. Enter the laser model name or model number in "Model"
  7. Select the laser class in "Class"
  8. Enter the Georgia Tech inventory number in "GT#"
  9. Select the "Department" to which the Laser Supervisor belongs
  10. Select the "Building" and "Lab" for the location of the laser
  11. Select the lasing medium in the "Laser Type" drop-down
  12. Enter the "Brief Description of Use" from the Form LR-1, Laser Registration in the "Application" field
  13. Enter "Yes" or "No" in the "Scanning?" field
  14. Enter the next sequential "ORS #"
  15. Enter the manufacture date in "Date Manufactured"
  16. In the "Laser Properties" section, Check "Continuous" for continuous wave (CW) lasers, or "Pulsed" for pulsed lasers
    • Click "+Add", then enter all of the laser property information provided in Form LR-1 into the proper field
    • For power or energy outputs, be sure to enter the value in units of W or J, the PRF in units of Hz, and pulse duration in units of seconds.
    • Click "Save"
    • Repeat this for each wavelength or each wavelength range that has the same output specs.
  17. In the "Custodians" section, check the box "Search By PI/Permit" and begin typing either the first or last name of the Laser Supervisor.  
    • Select the appropriate name from the list that appears.
    • In the box below, click "Attach" to attach the laser to the Laser Supervisor's permit
    • Click "Save"
