Each laser registered via Form LR-1, Laser Registration is then registered with the State of Georgia.

1. Visit the website https://forms.dch.georgia.gov/Forms/xrayv2, click on the "Form Selection" tab, and then select the "Laser Facility" radio button. Click "Next".
2. Fill out the form like shown in the following example.  The lasersepcific information is found in EHSA under Equipment, Laser.

   
   

3. Click "Submit".
4. Save a pdf of the confirmation email with the file name "ORS ORS#.pdf" in the server directory K:\Radiation_Safety\ORS Files\Lasers\GA Laser Registration Letters.  The ORS# for the laser can be found in the laser equipment record in EHSA.
5. Open the laser equipment record in EHSA and populate the "Georgia Registration Date" field with the date the online form was submitted.
6. Click "Save".
