1. Click on "PI and Permit Information" in the top menu
  2. Click on "PI & Permit Information"
  3. Find the row containing the PI for which you want to terminate a permit and click the small black arrow to the left of the last name.
  4. Double-click the row containing the laser permit you want to terminate.
  5. Click on the "Workers" tab and make note of the the names listed.  Each worker will need to be detached from the permit.
    • Click the row containing a worker name and click "Detach"
    • If training requirements are associated with this worker, a "Training Requirements" box will pop up.  If the only requirements are Laser Safety and Refresher Laser Safety, go ahead and click "Remove All". 
      • If there are other training requirements listed as well, click "Edit" instead. Under the "Training Requirements" tab, click the row that contains "Laser Safety" and then click "Delete".  Click "Yes" when the deletion confirmation box pops up.  Repeat this for the row containing "Refresher Laser Safety".  Leave all other training requirements alone.
      • Click the "Permit Edit" button to go back to the laser permit.
    • Don't forget to go the worker's User Record and delete "Laser User" and "Laser Supervisor" Security Groups (as applicable) and to sync the PI listing.
  6. Click on the "Labs" tab then click "Detach" next to each lab the appears to the right.
  7. Click on the "Lasers" tab.  DO NOT click the "Delete" button to remove the laser from the permit.
    • Click the row containing the laser and then "Edit Laser"
    • Scroll down to the "Cutodians" section and click "Archive" next to the Laser Supervisor under the "Current Custodians" tab.  Click "Yes" to confirm this archival.
    • While remaining on the "Current Custodians" tab, click the "Search By PI/Permit" checkbox just above the tab and select the new Laser Supervisor in the "PI" drop down menu.  The list of workers attached to this Laser Supervisor will display just below.  Click "Attach" next to the Laser Supervisor's own name.  If the new Laser Supervisor is not yet determined, select the current Georgia Tech LSO as the custodian.
    • Scroll all the way to the bottom of the screen and click "Save".
    • Repeat this item (7) for each laser listed in the "Lasers" tab.
  8. Click on the "Amendments" tab.
    • Click "+Add" and select "Amendment Type" "Termination"
    • Indicate today's date in the "Approved" field.
    • Select your name from the "Approved By" drop down menu.
    • Click "Save"
  9. Set the "Permit Status" to "Terminated".
  10. Click "Save".
  11. Archive the Laser Permit according to the instructions at https://www.ehs.gatech.edu/node/1605.
