Enter Amendment Record
- Open the Permit to which the Protocol will be added
- Click PI and Permit Information
- Under "Licensing Information", select PI & Permit Information.
- Find the PI and click the arrow to the left of their name in order to see their Permits.
- Select the correct Permit, and click Edit (or double click the correct Permit).
- Click the Amendments Tab
- If an Amendment was already entered for Interim Approval (Hint: look for an amendment with no approval date, or look for the Application's Title in the Amendment Description column), then double click it.
- Enter the Approved Date
- Under Approved By, select the RSO.
- If no Amendment was created yet, click Add
- Under Amendment Type select "Amendment"
- Under Amendment Description, enter the following: "Add New Protocol, [Approved Application Title]"
- Under Important Dates, enter:
- Requested - date the application was signed by the AU
- Received - date the final revised application was received by ORS
- Approved - date the RSO & RSC Chair signed
- Approved By - select the RSO
- Click Save
- Follow the next steps to scan the application into EHSA.
Scanning the Protocol Into EHSA:
- Scan the Approved application with RSO and RSC Chair signatures to their Form A file. Be sure file name says "...Approved".
- In the Edit Permit screen (see steps above), select the Amendment tab at the bottom of the page.and note the Amendment # for that corresponds to the protocol.
- Now, select the Protocol tab
- Enter information into the following fields:
- Corresponding Amendment Number
- Protocol –30 words or less, so abbreviate or condense the protocol title as needed
- Protocol Description (Full Application Title)
- Protocol Type (X-Ray or RAM Permit)
- Approval Date
- Under Attach Documents to Permit, click Add
- For Documentation Date enter the permit approval date
- For Type, select Approved X-ray or Ram Permit
- For Document Description, enter “[protocol name (see field above)] approved application”
- Click Select File under Document Name
- Select the file from Step 1, and click Open
- Save
