The individuals in this class have already completed RAM & Hands-on (for detection lab with Wang) 4 semesters earlier. They are adding additional AUs (Erickson, Grimm, & Hertel) and being issued dosimetry. Here are the things (in addition to standard sanity checking that those 2 forms have been completed) that need to be done for their Form Bs/Form Hs. If you have questions, ask Christina.

On the Form B:

1. Use the "Signature on File" stamp where the AU signature would normally go
2. Print the date that Christina gives you next to the AU signature line
3. Print "Erickson, Grimm, & Hertel" where the AU's printed name would normally go
4. Check the boxes for Film Badge and Extremity, and fill in their 5 digit Landauer participant ID into "Badge ID"
5. Check whether they have listed any prior exposure (within the last 12 months) in question 5
   1. If so, ensure we either have a copy of their prior exposure or have gotten them to fill out a prior exposure form (RS-114, should be attached to their packet). If there isn't one, fill one out with address and name, then give to Christina to get student to sign.
   2. If not, if we have already issued them dosimetry, check the "GA Tech" box
   3. If they haven't been issued dosimetry by anyone in the past 12 months, then check the "None" box
6. Check the box for "RAM" training, and complete the date they took initial RAM training (NOT refresher training, it's okay if the date is > 3 years ago)

On the Form H:

1. Check the boxes for Film Badge and Extremity, and fill in their 5 digit Landauer participant ID into "Badge ID"
2. In the "Comments", write "Ta badge"
