Preparation:
- Log in to EHSA at http://ehsa.gatech.edu
- Click on “Worker” then under the “Workers” tab click “Worker Application Import”
- Click on “Options” and then “Export To Excel”. This will open an Excel spreadsheet of the EHSA page
- Open up two other browser tabs for EHSA:
- On one tab, open “Workers” then under the “Workers” tab click “Workers”
- On the other, open “Equipment” then under X-Ray Devices click “X-Rays”
- Open the spreadsheet Worker Application Import.xlsx
- Sort the two spreadsheets by submitted date and compare to check for new Form B’s (ignore Laser registrations – category “LU-1”)
For newly submitted Form B’s:
- Click “View Applications” then “Submitted Application”, or double click on the worker to pull up their submitted Form B
- Look on the “I will be using” line to see if they are using RAM or X-Ray
- Look the person up in the Worker table and check if they have the X-Ray or RAM training needed
- If they don’t, this person has likely signed up for training
- If they do, then check “Attach PI” to see if they are already attached to the desired permit (see “Determining Permit” below)
- If they are not attached to the desired permit, and they are current on their training, then download and send the Form B via DocuSign to the worker and PI for signature (no PI signature is needed for IEN equipment. Just add “IEN. Signature on File” to the AU signature line)
- If they are attached to the desired permit, then this likely has been submitted unnecessarily by the worker. Click “Update Worker” –
- Compare the information in the "Import Value" column with the "Current Value" column and check the box in the "Change Value?" column if you wish to accept the new import value
- Click “Apply”
- An “Application Requirements” box will display. Click "Continue Edit"
Determining Permit:
- Using information on the Form B from the “I will be Using” and the “Laboratory: Building/Room/Department” lines, look in the Equipment -> X-Ray table to try to determine which AU’s permit to attach.
- Currently, the following AU’s all belong on the IEN umbrella permit: Berta X-151, Brown X-287, Henderson X-346, Suh X-288, Tavakoli X-486, Tian X-991, White X-393, Walters X-410
- If you’re still unsure, ask other ORS staff or email the worker and ask them (what equipment they plan to use? What building/lab/department they will be working in?)
