Form LS-1, Laser Supervisor Registration is submitted online via the link at Laser Documents | Environmental Health & Safety (gatech.edu). When these are submitted, the forms are available for import as described below.
Worker Application Import
- Go to "Workers">"Worker Application Import"
- Select "Pending" in the "View" drop down box
- Scroll to the Form LS-1 to be imported
- Double click the row for the Form LS-1 to be imported. This will open the pdf of the submitted form in another browser tab. This will be useful for part of the next steps.
- In the first column, click the green "Import Worker" button or the blue "Update Worker" link, whichever is showing.
- "Import Worker"
- The screen display will change and you should see "Worker Information" as the label at the top of the page.
- Fields that were filled out in the online LS-1 will be populated automatically.
- Proceed to Step 6.
- "Update Worker"
- A "Select Worker Information from Imported Data..." box will display
- Compare the information in the "Import Value" column with the "Current Value" column and check the box in the "Change Value?" column if you wish the accept the new import value.
- Generally speaking, accept changes where the "Current Value" is blank and the "Import Value" has information in it.
- Click "Apply"
- An "Application Requirements" box may display. Click "Continue Edit" without making any changes.
- Fields that were filled out in the online LS-1 will be populated automatically.
- Proceed to step 6.
- "Import Worker"
- If the "Position" field is blank, look at the pdf and type in the value that you see in the "Position Title" field. If that is blank, then enter the value from the "Employment Status" field. If there is not an exact match, look up the person in the Georgia Tech Directory, and use the position title listed in their entry instead of the one from Form LU-1..
- Click on the "Licensed" tab.
- Check the box for "Laser"
- Click "Save".
- Click on the "Licensed" tab.
- Click on the "PI and Permit Information" link on the top navigation banner and then click "PI & Permit Information".
- Click on the "+ Convert Worker to PI" button
- Type the name of the Laser Supervisor in the "Find Worker" search field and click "Search"
- Click the "Select" button next to the correct listing.
- An edit PI screen will pop up with the PI Code populated with the next sequential number.
- Scroll down to the "PI Status" field and select "ACTIVE" from the drop-down menu.
- Click "Save".
- Add a Laser Permit according to Add Laser Permit | Environmental Health & Safety (gatech.edu).
- Go back to the "Workers" menu, search for the person, and double-click the correct row.
- Click on the "Attach PI" Tab
- Find the row with the laser permit for this Laser Supervisor and change the value in the "Function" column from "Permit Holder" to "Laser User"
- Click "Save"
- Re-open the worker record
- Double-click the row for that person.
- Scroll down and click on the "Training" tab.
- Click on the "Training History" drop-down menu and now select "Required Courses".
- Do not manually enter training requirements.
- Ensure that "Laser Safety" and "Refresher Laser Safety" appear. If they don't, contact the LSO.
- Click on the "Training History" drop-down menu and now select "Required Courses".
- Click on the "Attach PI" Tab
- Click "Save".
- Click the "Add User Record" button. This will read "View User Record" if the person has previously been given a user record.
- Click "Sync with Permit Links". Ensure the Laser Supervisor name appears in the right column under this button or is already present in the left column under this button.
- Select "Laser Supervisor" in the "Security Groups" field.
- Click "Save".
- Generate a Laser Supervisor Registration Notification Letter. ADD SECTION FOR THIS AND LINK IT.
