Amend Permit to add new lab or isotope
- Add Amendment:
- Click the Amendment Tab
- Note the number of the latest amendment
- Click Add
- Select “Amendment” for Amendment Type
- Make sure the amendment number is the next sequential number to that found above.
- Under Category select either “Location Change” or “Radioisotope Change” as appropriate
- The category should appear in the box below, so click “Add to Amendment”
- Enter an amendment description
- Enter the Request Date, Received Date, and Approval Date
- Approved By should be RSO or ARSO.
- Save
If Adding a New Lab:
- Click the Labs tab
- Use the dropdown menu to select the building
- Find the lab in the list below and click Attach lab
- Click the Protocol tab
- Double Click the protocol for which the request was made (or click once and select Edit)
- Click the dropdown for Pick Location
- Select the lab that was added
- Click Add
- Save
- Click the Isotopes tab
- For each isotope allowed on the protocol,
- Select the isotope and click Edit
- Under Approved Locations, find the added lab(s) and check the boxes for Handling and Stored as necessary.
- Click Save
If Adding new Isotope:
- Click the Isotopes tab
- Click Add
- Click Select Isotope
- Select the correct isotope as appropriate for the units and the license (Broad or Exempt/General)
- Select Source Type
- Select Compound
- Enter approved Possession Limit. Possession limit can be the Shipment limit, too.
- If a Sealed Source is selected as the Source type, enter the Single Source limit under Sealed Source information.
- Under approved locations, find the added lab(s) and check the boxes for Handling and Stored as necessary.
- Click Save
- Click the Protocol tab
- Double Click the protocol for which the request was made (or click once and select Edit)
- Click the dropdown for Pick Isotope
- Select the isotope that was added
- Click Add
- Repeat for any other isotopes added.
- Click Save
- Put a copy of the written request and approval in the AU’s Form A folder in 3-76.
