Amend Permit to add new lab or isotope

  1. Add Amendment:
  2. Click the Amendment Tab
  3. Note the number of the latest amendment
  4. Click Add
  5. Select “Amendment” for Amendment Type
  6. Make sure the amendment number is the next sequential number to that found above.
  7. Under Category select either “Location Change” or “Radioisotope Change” as appropriate
  8. The category should appear in the box below, so click “Add to Amendment”
  9. Enter an amendment description
  10. Enter the Request Date, Received Date, and Approval Date
  11. Approved By should be RSO or ARSO.
  12. Save

If Adding a New Lab:

  1. Click the Labs tab
  2. Use the dropdown menu to select the building
  3. Find the lab in the list below and click Attach lab
  4. Click the Protocol tab
  5. Double Click the protocol for which the request was made (or click once and select Edit)
  6. Click the dropdown for Pick Location
  7. Select the lab that was added
  8. Click Add
  9. Save
  10. Click the Isotopes tab
  11. For each isotope allowed on the protocol,
  12. Select the isotope and click Edit
  13. Under Approved Locations, find the added lab(s) and check the boxes for Handling and Stored as necessary.
  14. Click Save

If Adding new Isotope:

  1. Click the Isotopes tab
  2. Click Add
  3. Click Select Isotope
  4. Select the correct isotope as appropriate for the units and the license (Broad or Exempt/General)
  5. Select Source Type
  6. Select Compound
  7. Enter approved Possession Limit. Possession limit can be the Shipment limit, too.
  8. If a Sealed Source is selected as the Source type, enter the Single Source limit under Sealed Source information.
  9. Under approved locations, find the added lab(s) and check the boxes for Handling and Stored as necessary.
  10. Click Save
  11. Click the Protocol tab
  12. Double Click the protocol for which the request was made (or click once and select Edit)
  13. Click the dropdown for Pick Isotope
  14. Select the isotope that was added
  15. Click Add
  16. Repeat for any other isotopes added.
  17. Click Save

     

  18. Put a copy of the written request and approval in the AU’s Form A folder in 3-76.
